Grove City Christian Athletic Forms
Athletic handbooks are located in the athletic office or can be downloaded from this website. Students must submit the following on or before actual tryouts; Athletic Agreement Form, Physical Form, Emergency Medical Form, Liability and Insurance Waiver Form, Transportation Form, OHSAA Authorization Form, OHSSA Eligibility Statement Form, Athletic Fee, and the OK to Try-out Checklist signed by the athletic director. Below is a brief description of each of these documents. The documents can be selected and downloaded from the column on the left.
OK to Try-out Checklist: This form is to ensure that all the student-athlete information, athletic fee, and parental acknowledgment of policies are in order. When complete this document will be signed by the athletic director. The packet will then be filed in the athletic office and a copy will be provided to the appropriate coach for the sport in which the student athlete is competing.
Athletic Handbook Agreement: All students and parent(s)/guardian must submit a signed Athletic Agreement Form acknowledging that they understand, agree to abide, and support the polices in both spirit and practice of the Grove City Christian Athletic Department.
OHSAA Forms: All students and parent(s) must submit an OHSAA Preparticipation Physical Exam, OHSAA Authorization Form, and an OHSAA Eligibility and Authorization Statement to the Athletic Department. Participation will be permitted upon completion. The physical examination is good for one calendar year.
Hold Harmless Agreements: All students and parent(s) must submit a signed Parental Permission and Transportation Form to the Athletic Department in order to be permitted to travel.
Risk of Participation: All students and parent(s) must sign a Liability and Insurance Waiver Form and an Authorization to Consent to Medical Treatment indicating an understanding of the inherent risk of participating in athletics.
